Elite Goalkeeper Institute – Liability Waiver, Assumption of Risk, Medical Consent & Photo/Video Release

Release of Liability and Assumption of Risk
I, the undersigned, acknowledge and agree that participation in soccer goalkeeper training at Elite Goalkeeper Institute (“the Institute”) involves inherent risks, including but not limited to physical injury, property damage, and other potential hazards. I voluntarily assume all risks associated with participation in training sessions and related activities.

I hereby release, waive, discharge, and covenant not to sue the Institute, its owners, directors, officers, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child while participating in training sessions or while on the premises of the Institute.

Medical Treatment Consent
In the event of an emergency, I authorize the Institute to seek medical treatment for me or my child and to arrange for transportation to a medical facility if necessary. I understand that I will be responsible for any medical expenses incurred.

Photo and Video Release Consent
I grant the Institute permission to photograph, video record, and/or otherwise capture my image or my child’s image during training sessions and related activities. I understand that these images may be used for promotional, educational, and marketing purposes, including but not limited to social media, websites, and printed materials.

I waive any right to inspect or approve the finished product or any use of such images. I release the Institute from any claims related to the use of these images, including claims for compensation, invasion of privacy, or defamation.

Acknowledgment of Understanding
I have read and fully understand this agreement. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
